Our reception staff are now trained Care Navigators
Care Navigators will ask for brief details before booking you an appointment.
Care Navigators are not being intrusive and are bound by the same rules of confidentiality as clinicians.
They are doing their job as requested by our GPs in order to offer a more streamlined and efficient service for you.
We are now able to register patients for online access.
We require all patients who wish to use the service to bring in two forms of ID to sign up. This is for security reasons as you will need to be given a new password for the new system.

Health & Social Care Information Centre Data Collection
The Health and Social Care Information Centre will be collecting data from patient records via a secure system. The information will be used to plan and improve NHS services for all patients. Information such as your postcode and NHS number, but not your name, will be used to link your records in a secure system by them so that your identity is protected. This will allow the HSCIC to compare the care you receive in one area against the care you receive in another to see what has worked best.
The data extracted from GP systems includes family history, vaccinations, diagnoses, referrals, biological values (such as blood pressure, BMI and Cholesterol) and all NHS prescriptions.
Sharing the information will help to
- Find more effective ways of preventing, treating and managing illnesses
- Guide local decisions about changes that are needed to respond to the needs of local patients
- Support public health by anticipating risks of particular diseases and conditions, and help us take action to prevent problems
- Improve the public's understanding of the outcomes of care, giving them confidence in health and care services
- Guide decisions about how to manage NHS resources fairly so that they can best support the treatment and management of illness for the benefit of patients
Your data will be handled in accordance to the law, national guidance and best practice. Only the minimum data will be extracted. Sometimes information will be released to approved researchers, if this is allowed there will be strict rules in place to protect your privacy.
Do you have a choice?
Yes. You have the right to prevent confidential information about you from being shared or used for any purpose other than providing your care, except in special circumstances. If you do not want information that identifies you to be shared outside your GP practice, please let us know and we can make a note on your records to prevent this.
If you are happy to have your data collected then you do not need to do anything.

The ground floor is designed to allow access to a consultation room and toilet. If you are unable to enter via the front door contact reception on 01706 253 700 and they will open the gates to access the rear ramp entrance.
If any patient registered with The Surgery has a disability, impairment or sensory loss we are able to offer you information that you can better access or understand, so that you can receive the necessary help you need. Please make a clinician or receptionist aware so we can update our records.
